Getting started is very easy. Simply fill out a Pre-Consultation Questionnaire. You can fill in a form on-line, or you can choose a form to fill out and print to send by fax or mail.
You will receive a follow-up phone call or email to discuss information which may be needed for your consultation (this will not count toward your consultation time). If no further follow-up is needed your consultation appointment will be set at an agreed upon time.
